Know Your Perscription Drugs

Make a list of drugs that have been prescribed for you and take the list with you to your physician, pharmacist, hospital and/or health care agency involved in your care.
Things you should know:
- Name of drug (generic or trade)
- Dosage
- Frequency (how often you take the medication)
- Why you are taking the drug
- Side effects
- How can you minimize side effects (such as taking the drug on a full stomach)
- Route taken (such as by injection, inhaler, or orally)
